Tattoos do not give special bonuses (a couple exceptions) and are not meant to make you stronger. If you have 10 extra gems and you want something cool, go for it. Like tattoos in real life, getting a tattoo at the parlor hurts! You know you'll feel pain for a few days (you'll loose some hitpoints at the beginning of the game day).
Note: If you no longer want your tattoo, you may get it removed by Sakura in the forest or the Ninja Hospital in Ninja Central.
